Myth versus evidence | Device accuracy
Imagine a monitor that has lived in the kitchen cabinet for four years. The display is bright. The batteries are fresh. Its readings look believable. That still does not answer the owner’s real question: “Is this unit measuring correctly today?”
The word calibration makes this harder than it needs to be. People use it for three different jobs. Separating those jobs gives you a practical plan and keeps you from testing one uncertain machine against another at the kitchen table.
Calibration, validation and an accuracy check are not the same
Validation answers, “Has this model passed an accepted performance review?” A recognized list should identify the exact model and the population for which evidence applies. The AHA recommends a validated, automatic, upper-arm cuff monitor and specifically says to make sure a device is validated for a senior, a pregnant person or a child when applicable.
An office accuracy check asks a narrower question: “Does this particular monitor agree closely enough with the clinic reference when we control the setup?” It also reveals problems that look like device drift but are really an incompatible cuff, wrong cuff size or a repeatable technique error.
True calibration is a technical adjustment against a known pressure reference. Some professional equipment has a formal calibration program. A consumer automatic monitor may instead have a manufacturer check, service route or replacement decision. Follow the exact manual. Do not open the housing, change internal settings or assume that clearing memory adjusts measurement accuracy.
What recent evidence adds to the annual-check advice
A 2024 clinic study tested 152 patients’ home devices with the AMA comparison protocol. Device agreement failed in 22.4% of tests. That does not mean one in five current monitors is universally inaccurate. It was a single-clinic sample, and the result was agreement under one defined office protocol. It does show why a plausible display and a familiar brand are not substitutes for checking the unit a person actually uses.
An earlier UK study of 331 patient-owned devices found 76% passed all monitor and cuff tests and 86% passed the static pressure test. Validated monitors passed the static test more often than unvalidated monitors, 96% versus 64%. The study used bench-style pressure testing and older devices from its own setting, so its percentages are not a shopping score for every model today. Its useful lesson is narrower: exact-model validation and cuff condition both matter.
Put together, the evidence supports a layered answer. Start with a validated model. Then check the individual unit in clinical use. Keep the cuff, tubing and connector in the decision instead of blaming the electronics first.
When should you check, service or replace a monitor?

What an office accuracy check actually looks like
Do not compare a home reading taken before breakfast with an office reading taken after driving, walking and waiting. Blood pressure changes with time and conditions. The equipment comparison needs to remove as much of that noise as practical.
The AMA tool is designed for a care setting. It calls for proper preparation and positioning, the same arm, and up to five seated readings in a defined sequence using the patient’s monitor and a validated office device. The calculation tells the care team whether the home unit agrees closely enough to use or should be replaced. It is not a hidden menu that changes the monitor.

- any approved second cuff you use;
- fresh batteries or the supplied adapter;
- the exact model number and manual;
- a recent, unedited reading log;
- notes about drops, error codes or changed readings;
- your question: “Can we check my unit and my technique?”
If your practice does not offer a device comparison, ask the manufacturer what service is available and where a qualified check can be performed. A pharmacy kiosk reading may be useful context, but it is not automatically a standardized calibration test for your home unit.

The inherited monitor with no manual
An older unit from a relative may turn on and still be a poor choice. If you cannot identify the exact model, confirm validation, find an approved cuff or obtain service guidance, do not make it the basis of a treatment decision. Replacement may be simpler than trying to prove an unknown setup.
The sound unit with the wrong cuff
A monitor can be functioning as designed while the system gives misleading readings because the cuff does not fit. Measure the middle of the upper arm and compare it with the stated range. The home-office gap

Frequently asked questions
Can I calibrate an automatic blood pressure monitor at home?
Only follow a calibration or service procedure that the exact manufacturer manual authorizes. Do not open the unit or use an improvised correction. For most owners, the useful step is an office accuracy comparison followed by manufacturer-directed service or replacement if it fails.
How often should I have a home blood pressure monitor checked?
The American Heart Association advises taking a new monitor to your next appointment and bringing it about once a year, or as the manufacturer directs, to make sure readings remain accurate. Your clinician or manual may set a different schedule.
Is a validated monitor guaranteed to stay accurate?
No. Validation supports the performance of an exact model under a recognized protocol. It does not guarantee that every individual unit, cuff, connector and user setup will remain problem-free. That is why an office comparison still has value.
Should I replace a monitor if it disagrees with the doctor’s reading?
Not from one ordinary pair of readings. Ask for a controlled, same-arm comparison that accounts for preparation, order, cuff fit and technique. Replace or service the unit when the care team’s protocol or manufacturer guidance indicates it should not be used.
Does resetting a monitor improve its accuracy?
A reset may clear memory, date, time, user profiles or connection settings, depending on the model. It does not by itself compare the pressure system with a known reference. Check the manual before resetting so you do not erase a useful record.
